I hereby confirm, by submitting this form, the verity of data shown above and I consent to the processing of personal data in the information systems of the company Slovenský plynárenský priemysel, a.s., Mlynské Nivy 44/a, 825 11 Bratislava 26 (further referred to as “Supplier”) to the extent required for concluding the Contract for the supply of CNG, creation of the Protocol of Training for the safe CNG filling method, and the delivery of information applying to introduction of the self-serviced CNG filling system at the Supplier s filling stations, either by ordinary mail, e-mail, by using other electronic means, or by personal delivery. This consent is granted for the period until conclusion of the Contract for the supply of CNG, as well as for the whole validity period of this Contract. This consent may be revoked in writing at any time.

I am hereby also confirming acceptance, from the Supplier, of the Information sheet on the self-serviced CNG filling.
